Download DSpeech for SAPI5 support! it's just as good as Speakonia

Of course it does bonehead how else do you think it can do this:
Speakonia is able to fetch webpages through an internal http interface and read them aloud for you.
Not all BHO's are undesirable, there are several programs that install their own BHO as part of their functionality:
Spybot Search & Destroy monitors your home page for unauthorized changes.
Roboform password manager keeps watch for login pages to pre-fill them for you.
Yahoo! Messenger adds a quick link to check Yahoo! mail.
